*Name for back of jersey. (If no name is provided and we cannot reach you, it will say "JONESBORO" across the top back.

*Special request such as color changes, design modifications, etc. We cannot guarantee that all requests will be granted, however we will communicate it before production. 

*If local pick up, please include pick up persons first & last name.

PLEASE NOTE THAT WE WILL NOT BE LIABLE FOR ANY INFORMATION THAT IS NOT PROVIDED SUCH AS WHEN YOUR ORDER IS NEEDED BY, COLOR CHANGES, ETC.
